A 52-year-old woman presented with a 1-year history of nonpruritic, non-tender erythematous papules on her face. She said she occasionally felt tightness and burning. The lesions initially responded to treatment with topical corticosteroids, but reappeared when the corticosteroids were discontinued. She was taking glyburide for type 2 diabetes and was allergic to sulfa-containing medications.

The most likely diagnosis is?

Gram-negative folliculitis
Gram negative folliculitis is an inflammation of follicles caused by a bacterial infection that can result from long-term antibiotic treatment. Patients who are being treated with antibiotics for severe acne may develop Gram negative folliculitis.

The word “Gram” refers to a blue stain used in laboratories to detect microscopic organisms. Certain bacteria do not stain blue and are called “Gram negative. Source

Gram-negative folliculitis is an acne condition caused by Gram-negative organisms. Usually people who had Gram-negative folliculitis are they who had complication with acne vulgaris and rosacea, also develops in patients who have received systemic antibiotics for prolonged periods. Source

The Bacillus Oleronius found by Dr Kavanagh in some demodex mites in rosacea skin, was gram-negative as well. Seems a popular type for infecting skin. I think they also classify them as lactose negative or positive, depending if the bacteria can grow on lactose. Considering I went lactose intolerant at the same time as developing papulopustular rosacea, I always wandered what happens to that lactose as it sits in my intestines undigested. Food for bacteria. Some bacteria must love lactose intolerant people. What if that bacteria has a way of getting to the face.
