    Default Permethrin Dosage

    What is a good starting point for Permethrin 5% treatment?

    The instructions say to treat only one time, but that is for scabies and crab lice. I have seen some that say once per week, but others that use it daily for months.

    It also says it is a corticosteroid, so would that present a problem if regular long term use was done?

    Permethrin is not a corticosteroid, it is a pyrethroid.

    I recommend daily as that works for me. I don't think I would have any success on a weekly dose.

    Thanks, I totally read the information packet wrongly. It says to use caution if you are using a corticosteroid...oops.

    I have read from several sources that a once weekly dose should be sufficient, but I really have no idea. I am very concerned that daily does would be overdoing it. Are there any good studies that give a good suggestion about dosage?

    This link suggests 1 weekly application, but that is after the infestation has been cleared. It seems like a good starting point, or maybe just once every third day for the first 30 days.

    The way I see it, the quicker the mites are killed, the quicker the die-off stage is over. If weekly works, then daily should work faster. If you also have type-1 rosacea, then a cautious approach may be better.

    I prefer the Dr. Bikowski recommendations:

    Cases of Demodex dermatitis can be treated with permethrin (Elimite) or crotamiton (Eurax) twice daily, morning and night, for 2 to 4 weeks. source

    There is quite a good summary of demodex dermatitis symptoms here by Dr. Bikowski.

    If you look at the treatment duration for the patients mentioned. Most are using the treatment twice a day for the first 4-8 weeks. Then reduced to once a day.

    The treatment used was crotamiton (Eurax) this time. Dr. Bikowski is paid to promote Eurax, I remember reading the disclosure in one of his other articles.

    Thanks for the info, this is very helpful. I have obtained both Lyclear and Eurax, but think I will go with the Lyclear for now.

    So after 2-4 weeks, what then? I saw you asked this question before, but I am really concerned about long term usage. Shouldn't something like once per week be good enough? How can they come back so fast? Other studies are suggesting once per week after the infestation has cleared.

    I don't know if this is helpful or not, but the duration of the topical Ivermectin trial here lasted four months. Once daily application at night.

    Significant improvements were seen within the first 1 to 2 weeks.

    I realise different drugs require different treatment periods. Just thought you might like to know the above though.
    Previous Numerous IPL, Sulfacetamide 10%/Sulphur 3%.
    Supplements: Discontinued High dose Vitamin D3, L-Glutamine (sometimes), Ascorbic Acid, Multi Vitamins, Tumeric, BHCL, Digestive Enzymes,
    Skin Care: Cetaphil Gentle Cleanser.

    Started CAP (combined antibiotic protocol for Cpn) on 9 Nov 2010.
    Discontinued due to rare and nasty reaction to metronizadole.
    Treating for gut infections under specialist care.

    Thanks for the tip. Wow, four months is quite a long time. What then? Is there a maintenance dosage?

    Although I have only applied the permethrin twice, it is interesting that I have not had any breakouts. I switched over from ZZ and things are getting better. I am not convinced the ZZ breakouts are really due to demodox.
