I have read a lot of posts but am still confused. Which laser would be best for my swelling and phymatous rosacea? How do I find a decent derm in my area? I'm feeling really discouraged.

Well, this can be tricky because many of the treatments cause a degree of tissue oedema (swelling) as the skin becomes plumped up with tissue fluid immediately after treatment. If a rosacea patient has a “swollen” face, I would consider using Intense Pulsed Light cautiously (after obtaining a full medical history to exclude other causes for the swelling). Every patient is unique however and advice should always be sought from an experienced laser physician who has had the opportunity to take a full history from and examine the patient before recommending treatment.
What treatment would you recommend for early onset of Rhinophyma?
I would generally recommend Intense Pulsed Light for early rhinophyma (depending upon the patient’s skin type and medical history). Early stages of rhynophyma react well to IPL plus oral doxycycline (oracea) and I supplement with Nd:Yag for the entire nose each treatment.

Some years ago, my IPL doctor had a patient who had phymatous rosacea on his forehead. It was really obvious and I kept visualising Herman Munster. I don't mean that in a nasty way. It was simply that protruding forehead which made comparisons in my mind.
IPL had little effect, so the doc used the Yag 1064 laser. Several treatments were needed. Maybe more, I can't recall exactly, but the results were wonderful.
You would never know this patient had previously suffered such a problem.
Now he has a maintenance IPL every 3 to 6 months.
This man had been told by his dermatologist he required surgery and not wanting to go that route, he decided to try IPL/Laser first.
Previous Numerous IPL, Sulfacetamide 10%/Sulphur 3%.
Supplements: Discontinued High dose Vitamin D3, L-Glutamine (sometimes), Ascorbic Acid, Multi Vitamins, Tumeric, BHCL, Digestive Enzymes,
Skin Care: Cetaphil Gentle Cleanser.
Started CAP (combined antibiotic protocol for Cpn) on 9 Nov 2010.
Discontinued due to rare and nasty reaction to metronizadole.
Treating for gut infections under specialist care.
