Dr. Nase,
Can accutane reduce the lumpy bumpy appearance of skin much like it reduces the inflamation and beginning stages of Rhinophyma? How can we detect whether fibrosis of this lumpy bumpy skin has set in? Is the result of this lumpy bumpy skin and fibrosis similar to that in rhinophyma?

Yes, lumpy bumpy skin without any notice papules or pustules is due to mild fibrotic changes in the skins surface that causes a ripple effect throughout the facial appearance. Accutane has moderate antifibrotic actions. This would be one indication for the photoderm with RF because that is more about treating those symptoms and skin remodelling than treating rosacea blood vessels.

Does fibrotic tissue tend to swell and decline given the amount of sebum being produced (non nose areas)?

Dr. Nase,

On what condition you reccomend a low dose of accutane before laser/IPL treatment or is it better afterward to maintain clearance from the treatments? I was thinking of going your route, 6 months at 10mgs and 9 months of 5 mg either before or after. If my memory serves, I believe that it is what you did when reading some post last year on the yahoo rosacea board. However, I wonder since I have permanent facial redness, micro-vascular matting, lumpy/bumpy skin texture, and scattered broken blood vessels, laser/IPL should be first to help clear the micro-vascular bed.
